§ 5.52.020. Application for license

Any person, firm or corporation desiring to operate such buses shall file in the office of the city clerk of the city of Centerville, an application for such license stating:

(a) The type of motor or jitney buses to be used;

(b) The horsepower and the factory number thereof;

(c) The state license number thereof;

(d) The seating capacity thereof according to its rating;

(e) The route; giving the names of the streets over which it is proposed to operate the bus or buses and the terminal at each end of the route; this shall be shown on a map, the map to be filed with the application;

(f) The age, name and residence of the person to be in immediate charge thereof as driver and a statement showing that such driver has attained the age of at least twenty-one years and if more than one person is to be in immediate charge of such jitney or motor bus that there must be given the name, age and residence of each of the persons and a statement showing that each of the persons had attained the age of twenty-one years;

(g) The qualifications and experience of the person who is to be driver of the jitney or motor bus and if more than one person is to drive same, that a statement of the qualifications of each of the persons be made;

(h) The names of the owner or owners of the bus or buses proposed to be operated;

(i) That a bond has been filed as provided by Sections 388.1—388.12, Iowa Code Annotated;

(j) The schedule upon which the bus or buses shall be operated.
